• Relevant and pertinent health information as prescribed in the Health Practitioners Competency Act 2003.

We collect your personal information in order to:

• Offer safe and effective Chiropractic Care.

Besides our staff, when applicable we share this information with:

• ACC to provide you with accident compensation.

Providing some information is optional. If you choose not to enter relevant and pertinent health information, we'll be unable to provide safe and effective Chiropractic Care. We keep your information for ten years from the last appointment with our office at which point we securely destroy it by.

You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong. We keep the personal information we hold safe by storing it in encrypted files and only allowing certain staff access to it.

We keep people's personal information for 10 years from the date of their last appointment, at which point we securely destroy it digitally.
